How do I make appointments?
If you would like to make an appointment, please call our office at 309-263-8317 .
Our regular hours are Mon. - Thu. 7am-4pm
What should I bring to my appointment?
Please bring:
1.Your online patient forms that were emailed/texted to you or completed on the website's home page
2.Your current DENTAL insurance card to your first appointment
3. If you have prevous x-rays (within five years) please have them emailed to our office (full mouth, panorex, and/or bitewings)
What to expect on your first visit?
Initial Exam: Oral cancer exam, TMJ and occlusion exam, hard and soft tissue check.
Cleaning: If you would like a cleaning in addition to your initial exam, please specify when scheduling.
X-rays: Full mouth x-rays (if you have not had a panorex or full mouth x-rays taken in the last five years) or bitewings (cavity detecting x-rays).
